This is the current version (as of 7/2009) of the Massachusetts Registry of Motor Vehicles crash form. You may fill out the form and submit it or pick up blanks at any police department.
The form should be mailed to the Registry, the police department where the accident occured, and your insurance company. Additionally, you should keep a copy for your own records.
If the Westwood Police covered the accident, you can request a copy of our report using the request a report form. You can request a copy of the other driver's accident form from the RMV Here.
Accidents on Route 95/93 are covered and should be sent to the Mass State Police Framingham Barracks.
